To support whole-person and patient-centered care, IMBH’s case management and care coordination approach promotes behavioral health care that is aligned and integrated with medical care as well as medical case management. It is often difficult for patients to navigate the healthcare system and complexities of health insurance.  Our team helps members access appropriate providers, address gaps in services, and coordinate care. These efforts minimize illness progression and promote better health outcomes. 

Identifying Target Population

Modeling guidance with health plans to help identify high-utilizers, complex and chronic cases, and those patients with co-morbid medical conditions.

Peer Support

Peer support and other wrap-around services help connect members with local resources to help manage their behavioral health recovery.

Elder Care Case Management

Patients 65 years old and older may need extra support to manage their changing circumstances.

Patient Monitoring

Measurement-based care tools via web and app report patient outcomes to IMBH allowing early detection of any decline.

Supportive CBT

IMBH offers a web-based cognitive behavioral therapy (CBT) to provide extra support when patients are in therapy.

Levels of Case Management Based on Acuity

IMBH team includes licensed clinicians, social workers, and community health workers to provide in-person, home, and telephonic case management depending upon need.
